Whenever you've been to a doctor's office, you've had to fill out new patient intake forms. Our Chiropractic clinic in Houston has its own forms too; they are very necessary and must be filed with us on the day of your appointment.

Print & Fill 'em In At Home!

It's not required, though printing and completing our new patient forms at home before visiting our office will save you time and help make your first appointment fast and easy.

Simply download the form(s) which apply to you, print and bring the completed form(s) with you on your visit. Both the New Patient Information and Pediatric Medical History Forms are Adobe Acrobat format. You'll need the free Acrobat Reader to view and print them.

Should you have a problem, please call our office; we'll be happy to mail, or fax them to you.
